I understand that the ThoughtFull Application is a mobile based-text messaging platform for online mental health coaching with licensedand registered mental health professionals.
By granting my child/ward access to the application, I amproviding my consent (and my child/ward’s consent) to:
1. providing my and/or my child/ward’s personal information for the purposes of using allThoughtFull services;
2. additional terms governing the use of the services, such as theTerms of Use (available at https://www.thoughtfull.world/clients-terms-of-use), including the disclaimer and limitation of liability clauses therein;
3. Thoughtfull’s Privacy Policy (available at https://www.thoughtfull.world/privacy-policy); and
4. The disclosure of relevant information in extraordinary situations where applicable law/professional ethical standards requires mandatory reporting in certain events (e.g., where human life is endangered or if there is suspected ill treatment, neglect, or abandonment).
Further, I confirm that I understand and agree with the ThoughtFullApplication (and professionals working with Thoughtfull) being subject tovarious regulations/professional ethical standards which, among others, requirethe information on the application to be strictly confidential to the client(i.e. my child/ward).
I acknowledge that Thoughtfull’s provision of services willconstitute acceptance and a binding agreement will be formed.
